"Right Care Right Here"

"Right Care Right Here" is one of England's most ambitious programmes to change the way that health and social care is delivered and it is happening here in Sandwell and the Heart of Birmingham. A range of services has already moved from the hospital out into the community, thus making it more accessible and convenient to the patient. The PCT has been working closely with transport providers to make sure that bus routes suit patient needs.

"Right Care Right Here" has introduced five community specialist clinics for diabetes in Sandwell and the Heart of Birmingham where patients can receive specialist treatment, advice and support on how to manage their condition. They are seen by a range of specialist diabetes staff including a Consultant Diabetologist, Diabetes Specialist Nurse, Specialist Dietician, Podiatrist and Health Care Assistants. This empowers patients to make informed choices by increasing their understanding and knowledge of diabetes.

Balsall Heath Health Centre

Birmingham Treatment Centre, City Hospital

Tower Hill Medical Centre, Great Barr

Coventry Road Medical Centre, Small Heath

Greet Health Centre (to open later for diabetes services)

* these centres have specialist eye services in addition to their other diabetes clinics and from September 2009 Aston Health Centre in Trinity Road and Rowley Regis Hospital will join them.
